Prudential Spirit of Community Awards 2019
The Prudential Spirit of Community Awards program is the United States’ largest youth recognition program based exclusively on volunteer community service.
Created in 1995 by Prudential and the National Association of Secondary School Principals (NASSP), the program honors middle level and high school students for outstanding service to others at the local, state and national level.
Thus far, it has recognized more than 125,000 young people who’ve made a difference—and inspired countless others to consider how they might contribute to their communities.
The national event in Washington D.C. will take place May 4–7, 2019 to honor the achievements of the 102 State Honorees and to name 10 National Honorees.
Read over the following documents and forms found in steps 1–4. The forms in the first two steps must be completed and returned in the appropriate envelopes as soon as possible, but no later than Tuesday, February 26, 2019.
Step 1: Please complete, print, sign, and return the three documents below to NASSP in the envelope you received in the mailing.
Step 2: The W-9 needs to be filled out in the honoree’s name. Please complete, print, sign, and return the document below to Prudential in the envelope you received in the mailing.
Step 3: The following are provided to help you prepare for your trip to Washington, D.C., and should be read in their entirety as they will likely answer many of your questions.

Step 4: Attention honorees and parents/guardians: You must complete this form with important information for the Prudential recognition program in one sitting by Tuesday, February 26. If you exit or do not complete it in totality, you will need to start again.
You will need the following information to complete the form:
- Birthdate of honoree and all attending adults (month, date, year)
- Jacket and t-shirt size of honoree
- T-shirt color preference (red, orange, or yellow)
- Dietary and any special needs
- Travel logistic preferences
Important Travel Information
Travel, hotel, and program expenses for your child and one parent/guardian will be paid for by Prudential. If a second guest is attending with the honoree and parent/guardian, the second person must pay a $500 registration fee, which does not include airfare or train fare (see Reply and Acceptance Form).
If the honoree is traveling with one parent/guardian you will not be contacted by our travel partner, Conference Direct. Once this travel information form is received in our office, we will automatically proceed with secure travel arrangements for you according to the program schedule.
Tickets will be issued with a confirmation delivered electronically to the email addresses provided in the form. Additionally, a printed confirmation of your tickets will be included in your next mailing (arriving the first week of April 2019).
Please note that tickets are non-refundable and non-transferable. Any changes once airline or train tickets are issued will incur imposed penalties and will be the sole responsibility of the traveler(s). This includes any travel upgrades as all tickets are booked in coach/economy.
